Hospital Admission

If you, a family member or friend are currently an inpatient at another facility seeking to transfer to Rancho for inpatient rehabilitation and/or specialized medical services call or e-mail us at our Centralized Admission and Referral Office. Our professional staff can help determine if our services will meet your rehabilitation needs. All information will be kept strictly confidential.

Admission Referral Form
Submit a completed admission referral form on an individual (patient) that would benefit from admission into Rancho for inpatient medical rehabilitation. We'll contact you directly to discuss the admission. If you are in need of urgent or emergent care, please call 911.

Are you currently an inpatient at another facility?

Your doctor or health care professional may refer you if you are currently an inpatient at another facility seeking to transfer to an inpatient rehabilitation and/or specialized medical program.

Depending on your geographic location, our nursing staff may go directly to the hospital to evaluate you for inpatient medical rehabilitation services or you will be contacted by telephone.

If you are not currently an inpatient at a medical facility, but are seeking to be admitted, select the link for information regarding an outpatient evaluation for admission.
