Student Orientation Materials

Welcome Students! Please complete student orientation forms online at: http://dhs.lacounty.gov/cef. Bring your driver's license, passport, or DMV Identification to your scheduled Live Scan appointment.

1. Employment Forms

Please complete student orientation forms online at: http://dhs.lacounty.gov/cef.

2. Health Clearance Forms

Complete and submit all forms in the packet.

1. Non-DHS/Non-County Workforce Member Health Clearance Certification - The first page needs to be completed by your doctor or a licensed health care professional. Forms E2 and B-NC are required. Form E2 needs to be signed by the school personnel who will have access to the records in case of an audit. Submit K-NC Declination form if you are declining any immunization.

3. Trainings and Orientation

Complete all trainings and orientations prior to your first day. Bring completed quizzes/acknowledgments on your first day.

1. Compliance Awareness Training - Log-in with your name to complete the online training. This training will take between 60-90 minutes to complete. Print and submit the completed Code of Conduct acknowledgment form at the time of your live scan.



2. Code of Conduct Training - Review and complete the first two lines at the bottom of the Code of Conduct acknowledgment form at the end of the training.



3. HIPAA Online Training - Login with your first and last name. Review and take the quiz found at the end of the training. Print and submit the Patient Privacy & Confidentiality Acknowledgment.



4. Orientation Review Handbook - Please review. Complete the Hospital Orientation Review Attestation and submit it to your department contact for processing.



5. Infection Control Orientation Self-Study - Please review and submit a completed post test found at the end of the guide.



6. Risk Management Employee Handbook - Please review and submit a signed acknowledgment form found at the end of the document.
